Brooke Mueller's Crack Binge -- And $1500 Meth Drug Deal -- Caught On Video

This is the reason why Charlie Sheen's ex-wife Brooke Mueller lost custody of their two children.

In a shocking video and series of photos obtained exclusively by RadarOnline.com, the troubled Palm Beach socialite and ex-reality television star is caught on camera smoking crack cocaine out of a glass pipe and scoring $1500 worth of crystal methamphetamine.

The emergence of the horrifically graphic scenes underscore why a Los Angeles judge, on the advice from the Los Angeles County Department of Children & Family Services, removed the hopeless addict's twins, Bob and Max, aged four, from her custody in May.

Investigators had fears the mother-of-two's rampant drug use could endanger the boys.

The video shows a haggard looking Mueller -- who is today fighting to remain clean and sober during a staggering 20th stint in rehabilitation -- clutching a glass pipe.

Dressed in a brown robe, she uses a lighter to ignite the crack, a freebase form of cocaine, then deeply inhales the drug.

"Where is the crystal? I am giving you $1500," Mueller is heard on the tape telling her drug dealer, before repeating those words for a second time.

It was purportedly shot before she lost custody of the children.

The source of the drug binge video has come forward in a desperate bid to save her life and beg Mueller, 36, to stay clean and complete her current rehabilitation... before it's too late.

“Brooke loved getting high," the source said. "This was just another crazy night of her getting high.  When she did drugs, it was three to four times a week.

"I know that she is aware that if she doesn't stop doing drugs, she is going to lose her kids (for good)... hopefully she can stop."

We broke the story: Mueller was placed on an involuntary psychiatric hold after overdosing on crystal methamphetamine, a highly addictive psychostimulant.

It was her sixth overdose in the last few years, as RadarOnline.com previously reported.

Mueller -- who starred in the 2011 Oxygen series The World According to Paris -- also regularly used other drugs including the prescription drug Adderall, before her most recent treatment, sources have told RadarOnline.com.

"It's amazing Brooke is still alive," a source told RadarOnline.com earlier this year.

"The only thing that has saved her when the overdoses occur is that someone has always been with her and able to call 911... She doesn't get how lucky she is to still be alive."

This is not the first time Mueller's drug use has been exposed: In May, 2011, Sheen released exclusively to RadarOnline.com a series of images which he said showed his ex-wife's supposed home made crack pipe, foil, spoons, drug residue and Mueller's driver's license which he'd discovered in the bathroom of a Bahamas resort.

In a seemingly unorthodox move, the Los Angeles judge who stripped Mueller of custody placed the twins in the care of Sheen's second wife, Wild Things actress Denise Richards with the blessing of the Anger Management star.
