That's the shocking claim of a respected doctor who viewed these disturbing photos and video of the beloved star, taken during his treatment at Minnesota's Hazelden clinic.

RadarOnline.com obtained the jaw-dropping material from an insider who received treatment for alcoholism alongside Chase at the respected rehab facility this fall. The Caddyshack star admitted he was back in treatment in September.

A source whose loved one was treated alongside Chase told RadarOnline.com, "My boyfriend told me that Chevy is very disheveled, unshaven, and riding one of those motorized scooters all the time because of his health. It doesn't look good!"

"When he came in, he was in the hospital for three days for detox, instead of just one day, which most people there do," the source claimed. "He's very sick."

According to the source, Chase, 73, opened up about his chilling health problems in a heart-to-heart with another patient. She claimed, "Chevy said his liver enzymes are elevated from the alcohol abuse," which can be a warning sign of cirrhosis.

NYC internist Dr. Stuart Fischer, who has not treated Chase, told RadarOnline.com, "Being as overweight as he is, combined with drinking so much that he had to check into rehab, has put his health in dire jeopardy."

If he doesn't get a transplant or change his behavior, Fischer said, "This could very well be the last year of his life!"

"His liver damage is likely caused by what we call 'pate syndrome,'" Dr, Fischer claimed. "That means he's collected fatty deposit in the liver from being both obese and alcoholic. It's a double whammy for the liver. It can lead to cirrhosis, liver cancer or even death."

Chase has struggled with addiction for decades. According to reports, he started popping painkillers back during his days on Saturday Night Live. In 1986, he entered rehab at Betty Ford. Then this fall, he was back again.

But according to the source, Chase still doesn't seem that concerned. She said, "My boyfriend told me that Chevy isn't very well liked in there, because he is joking around constantly and doesn't seem to be taking it seriously." Indeed, he was seen wearing a shirt that said "Do Not Resuscitate." The source continued, "He flirts with the girls and plays a lot of pool too."

But Dr. Fischer was adamant that Chase's health is no joke: "If he continues treating himself this way with poor diet and alcohol abuse, the curtain will come down on his act forever!"
