Jenelle Evans was rushed to the hospital this weekend with a nasty head injury, RadarOnline.com has exclusively learned.

According to the source, on Saturday night, "Jenelle was going out to dinner with her boyfriend David and friend Tori, and when she got home, everything was flooded on her street and she slipped and fell on the driveway getting out of the car."

"She busted her knee open," as well as her chin, the source said. “The whole reason Jenelle fell and slipped was because of the bad weather and her entire street was flooded."

Rushed to the hospital shortly after midnight, she "had to get stitches on her chin" and was released shortly after 7 am.

The reality star and mother of sons Kai, 15 months, and Jace, 1, is now "tired and in pain," according to the insider, but on the mend.

The accident comes on the heels of a stretch of good luck for the Teen Mom 2 star. After a nasty breakup with her ex, Nathan Griffith, and recent arrest after an incident involving his girlfriend, she has rebounded with David Eason , as RadarOnline.com reported.
