Catherine Zeta-Jones has "proactively" entered a rehab treatment facility to manage her bipolar disorder, a spokesperson for the actress confirmed to RadarOnline.com.
"Catherine has proactively checked into a health care facility," the rep said. "Previously Catherine has said that she is committed to periodic care in order to manage her health in an optimum manner."
This isn't the first stint in a treatment center for Michael Douglas' 43-year-old wife, who checked in to a hospital two years ago.

It's unclear where Catherine is receiving her reported 30-day treatment this time, but in April 2011 she stayed at the Silver Hill Hospital in Connecticut.
“After dealing with the stress of the past year, Catherine made the decision to check in to a mental health facility for a brief stay to treat her Bipolar II Disorder,” a rep for Zeta-Jones said in a statement, at the time.
