Nov. 8 2016, Updated 6:18 p.m. ET
Two years after Terrell Owens' ex-wife attempted to commit suicide, she landed back in the hospital with stab wounds to the neck. An incident report exclusively obtained by RadarOnline.com reveals Rachel Snider tried to take her own life for the second time.
According to the Beaumont Police Department's incident report, police responded to Snider's Texas home on October 20, 2016.
"A citizen attempted to commit suicide," the report read. "An emergency peace officer's committal was completed."
A "knife/cutting instrument" was found at the scene.
Although the incident report didn't disclose the exact injury, a source close to the 36-year-old claimed she was stabbed in the neck.
"She has more than a dozen stab wounds in her neck and had to have major trauma surgery," the insider alleged, explaining she recovered in the ICU before her October 30th release from the hospital. "She might have to have another surgery."

Soon after the incident, Snider posted cryptic tweets, along with a photo of her bandaged neck, hinting that she tried to take her life.
"Almost dying is awfully easy. It's the living that's hard," she posted on October 21, adding on October 29, "So go ahead. Fall down. The world looks different from the ground."
But despite the evidence, the source claims she didn't try to take her life.
"A family member did this," the source said. "It wasn't a suicide attempt."
The hospitalization comes two years after Snider attempted suicide two weeks into her marriage to Owens in February 2014.
"Everyone speculated it was over Terrell but it wasn't," the insider said of the initial attempt. "It had nothing to do with him at all. It had to do with a lot of things he wasn't part of."
