A former beauty queen filed a restraining order against ViSalus Sciences CEO Ryan Blair and alleged the entrepreneur and author broke her nose and knocked out her teeth, according to the filing obtained exclusively by RadarOnline.com.
In explosive documents filed Tuesday, Kasie Head said Blair had continuously physically, verbally and emotionally abused her over the course of their relationship. She said Blair was recently committed for psychiatric treatment for what she describes as a psychological breakdown.
"As set forth below, Ryan has broken my nose, knocked out my teeth, punched me in the mid-section and choked me," Head said in her declaration. "The incidents of domestic violence span over a decade, and continue despite our permanent separation in 2014.
"Ryan's recent conduct prior to his hospitalization is disturbing, threatening and frightening. I worry that he will harm me when he is released, which could be any day."
Blair, 40, became the CEO of ViSalus Sciences in 2005 and took the multi-level marketing company from bankruptcy to more than $600 million in revenue. He released his autobiography "Nothing to Lose, Everything to Gain: How I Went from Gang Member to Multimillionaire Entrepreneur" in 2011.
Head, 36, who was Miss Oklahoma in 2002, also asked protection for their 8-year-old son, Ryan Reagan and asked the court to order Blair to keep at least 150 yards distance from her and their son.

Head said she isn't sure what caused Blair's breakdown, but added his conduct became more extreme after the death of his mother in 2017.
Head also said she is concerned because she found pictures in her son's iPad of several large guns, including an assault riffle. She alleges those pictures were taken during Regan's trip with Blair in Arizona.
A judge approved Head's request for the temporary restraining order and ordered Blair to attend mediation on Feb. 28.
