Herman Echevarria's tragic death happened suddenly, RadarOnline.com has learned.

Echevarria's apartment building's manager found him unresponsive on September 26 after two of his friends called the building for a "wellness check" on him.

"They went up to the apt. and knock on door and after no response, he obtain written notice from deceased son to enter apt. with master key sic," the bombshell incident report obtained by RadarOnline.com read.

"W1. stated he entered apt. with W2. & W3.aAnd they discovered the occupant laying in bed," the papers continued. "At which time W2.attempted to wake occupant up moving the body."

Echevarria's last communications with his friends were two and four days before his death when his prescription medicines were being dropped off. He was laying in bed on both occasions.

His estranged wife, Alexia, was not present when he was found, but reported to police that he "had a lot of medical issues and was not feeling well over the last few days" prior to his death.

The Miami coroner later confirmed his death was due to natural causes.

Echevarria, 61, appeared on The Real Housewives of Miami alongside his then-wife Alexia, 49. They separated shortly after the show went off the air.

She released a statement at the time of his death via Instagram.
