Luke Perry's Death Certificate Revealed: Actor Buried In Tennessee

Luke Perry, who died on March 4 after suffering a massive stroke at age 52, has been laid to rest in Tennessee, RadarOnline.com can report.

According to the Beverly Hills, 90210 actor's death certificate obtained by RadarOnline.com, he was buried at the Taylor Funeral Home and Cemetery in Dickson, Tennessee on March 11.

Perry, who was born in Ohio and later relocated to Los Angeles, owned a $1 million property in Tennessee up until his sudden death, RadarOnline.com can report.

As indicated on the certificate, a funeral service took place at the Alpha Society in Burbank, California, on March 7, just days before he was buried across the country.

The certificate lists his final cause of death as "ischemic cerebrovascular accident."

As RadarOnline.com reported, medical personnel rushed to Perry's Sherman Oaks home after he suffered a stroke on February 28. In the chilling 911 call, featured above, a female caller begged officials to "hurry up." He was then taken to a nearby hospital, where he died one week later.

Perry left behind two children he shared with his ex Minnie Sharp. At the time of his passing, he was engaged to actress Wendy Madison Bauer. The two had plans to tie the knot on Aug. 17, 2019, RadarOnline.com confirmed.

The death certificate listed Bauer as Perry's "health care directive."
