“Marin County Communications received a 9-1-1 telephone call reporting a male adult had been located unconscious and not breathing inside his residence …” the report reads. Williams was pronounced dead at 12:02 p.m.

“An investigation into the cause, manner, and circumstance of the death is currently underway,” the report explains, but “at this time, the Sheriff’s Office Coroner Division suspects the death to be a suicide due to asphyxia…”

The Mrs. Doubtfire star was last seen by his wife at 10 p.m. the night before, the report claims.

After a lifetime of substance abuse issues, Williams entered rehab again last month, at The Lodge at Minnesota’s Hazelden treatment center, reportedly to “focus on continued commitment” to his sobriety.

He also spent time in rehab in Oregon in 2006 after falling off the wagon, after two decades of staying sober.
