Once a hot mess, always a hot mess!

Former Boardwalk Empire bad girl Paz de la Huerta has maintained a relatively low profile lately, but showed up on the red carpet of a Miami event on Wednesday night up to her old antics and exhibiting bizarre behavior, and RadarOnline.com has photos and exclusive eyewitness details.

Wearing a skintight black top with a purple tube skirt with blood red lipstick painted outside the lines of her lips, the actress struck a variety of provocative poses at the VIP grand opening of the H&M Lincoln Road store, where the exclusive guest list included Flo Rida and Chase Crawford, but it was inside the event that Paz really garnered attention.

"She did not at all look like she was in her right mind," an onlooker exclusively told RadarOnline.com. "People were making snickering comments about her all night."

And it wasn’t just her behavior that had people talking! At some point in the evening, she suffered an extremely embarrassing wardrobe malfunction, and didn't seem to notice even though everyone else did.

"On top of her somewhat dazed demeanor, she had a very noticeable hole in the back of her skirt."

This isn’t Paz’s first time making a scene at a red carpet event!

At a Golden Globes after party in 2011, she was infamously caught on camera getting denied access to a super swanky soiree, proceeding to fall down and tear her designer dress, flashing her breasts to the crowd.

Months later, she was charged with assault after a reported barroom brawl with The City star Samantha Swetra. She pleaded guilty to harassment and her sentencing included community service and alcohol counseling.
